Fainting is not a routine tolerability problem. New fainting, near-fainting, a fast or irregular heartbeat, or severe dizziness during treatment needs prompt assessment. Call emergency services when symptoms are severe, accompanied by chest pain or breathing difficulty, or cause collapse.

QT risk comes from combined vulnerability

The QT interval represents part of the heart’s electrical recovery. Azithromycin can prolong that interval, and rare dangerous rhythms become more plausible when several risk factors coincide. Known long QT, prior torsades, marked bradycardia, uncompensated heart failure, low potassium or magnesium, older age, and concurrent QT-prolonging drugs deserve attention.

Vomiting, diarrhea, diuretics, or other conditions can disturb electrolytes. A person may not know the current values without testing. The clinician decides whether history is sufficient, an ECG or laboratory check is needed, or another antibiotic avoids unnecessary cardiac uncertainty.
